HOW IT WORKS

OBTAIN A REFERRAL

Speak with your physician about writing a referral for Occupational Therapy to do a Home Safety Assessment. This can be faxed to (855) 952-5782. We can help you with this process if you would like.

Once the referral is received, we will send you intake paperwork that must be completed and returned. This will include a release of information so we can get any related medical information from the physician to be prepared for our visit.

After we receive all the necessary completed paperwork, we will call to schedule an appointment to come to your home.

MEET WITH YOU

We will meet with you in your home to learn about you and your goals.

We will first discuss what your current health issues are and determine what concerns you have in your home.

Our goal is to assess what problem areas there may be that could potentially cause safety concerns.

WALK THROUGH THE HOME

We will ask you to demonstrate your ability to maneuver throughout the home.

You will be asked to show how well you can transfer on and off different surfaces such as your chairs, bed, toilet, shower, etc.

We will need to see how well you can access all necessary items in your home.

PLAN FOR MODIFICATION

After we have completed the physical assessment, we will discuss any problem areas or areas of potential concern.

We will make recommendations for plans for modifications which could be as simple as removing throw rugs or improving lighting, or as complex as basic remodeling.

We will recommend any adaptive equipment, such as reachers or shower chairs, that may assist with improving your overall safety.

We will discuss the results with you and your family. A complete report will be sent to your physician.
