OBTAIN REFERRAL

Speak with your physician about writing a referral for an Occupational Therapist to perform a Comprehensive Driving Evaluation. This can be faxed to (855) 952-5782. We can help you with this process if you would like.

Once the referral is received, we will send you intake paperwork that must be completed and returned. This will include a release of information to get any related medical information from the physician to be prepared for our visit.

After receiving all the necessary completed paperwork, we will call to schedule an appointment to come to your home.

CLINICAL TESTING

This will be performed in your home or somewhere in your community that is comfortable for you.

We will formally evaluate your physical function (strength, range of motion, sensation, vision, how well you move) and your cognitive function (how your brain works, thinking and concentration).

The clinical testing will include both physical and mental tasks.

A family member is encouraged to observe this part of the evaluation.

BEHIND THE WHEEL

If everything goes well in the clinical testing, we will proceed to the Behind the Wheel testing.

We will need to use the company vehicle for liability purposes as it has been modified with an instructor brake for increased safety.

During on the road assessment, you will drive in both familiar and unfamiliar areas.

We will be monitoring your speed, lane position, acceleration/deceleration smoothness, and how well you adhere to the rules of the road, for example.

Family is NOT allowed to accompany you on the Behind the Wheel Assessment due to liability purposes.

OUTCOMES DISCUSSION

At the end of the session, we will sit down with you and at least one other family member and talk about how you did.

*Generally there are 4 possible outcomes:

1. You can continue to drive without restrictions.

2. You may continue to drive with restrictions such as no night driving or no driving in bad weather.

3. You may need further training sessions before a final decision is made.

4. It may be recommended that you retire from driving, at which time we will try to assist you with alternate plans for your transportation needs.

*A complete report will be sent to your physician.
